Student Registration Form

Welcome.

Please complete the below registration form which includes a short health questionnaire. It is mandatory that this form is completed prior to practising with Jodi Evans Yoga.

The completion of this form is binding in conjunction with the privacy policy and terms and conditions set out here. All information will be kept in the strictest confidence.

Terms here are to be read in conjunction with the terms, conditions and privacy policy set out on the website (full terms & conditions)

  • Arrive at class at least five minutes early to settle and be ready to begin on time.

  • If there is the occasional need to arrive late or leave early please be quiet, considering other students.

  • Always inform the teacher of any new or existing health conditions/injuries prior to practice.

  • If you do have any prior health concerns be sure to consult your Doctor and have them give the go ahead to practice.

  • Be sure to listen to your own body and not practice beyond your capacity. Participation is at the risk of the student, any damage, loss or injury, (mental, physical or emotional) is not the responsibility of the teacher.

  • Wear comfortable clothing that allows the body to move freely, and feet are bare.

  • Bring your own non-stick yoga mat to practice, (camping mats are not acceptable), however if you need to borrow a mat for the first week or two, contact Jodi to make arrangements.

  • Yoga is a practice of going inwards, be gentle and kind to yourself and please enjoy your experience.
